Waiver, Release of Liability, Assumption of Risk & Medical Consent

Voluntary Participation & Risk
Participation is voluntary and involves inherent risks including injury, illness, disability, death, or property damage.

Assumption of Risk
I knowingly and voluntarily assume all known and unknown risks associated with participation, including those arising from ordinary negligence.

Release, Hold Harmless, and Covenant Not to Sue
I agree to release, hold harmless, and covenant not to sue Day One Sports Performance, LLC. This release applies only to claims of ordinary negligence and does not apply to gross negligence, reckless conduct, or willful or intentional acts. This agreement is binding upon my heirs, executors, administrators, assigns, legal guardians, and estate.

Medical Consent
I consent to receive emergency medical treatment deemed advisable in the event of injury, accident, or illness.

Photo & Video Release
I grant permission for my likeness to be used for marketing or promotional purposes unless I indicate otherwise in this form.

Acknowledgment
I understand that signing this agreement affects my legal rights. I have read this document in its entirety, understand its contents, and agree voluntarily.
